However there's a few instances where the term "mom" is hard coded into the dialogue instead of using the tags for what the player chose. Just a tip to maybe fix. An example of the top of my head is in the card game at the inn. She calls her your mom no matter what you chose.

No, your game is not bugged. The only way to get there is by Darja taking you. If she didn't take you then it's because you already have a lair. You can have the Ancient Spire, Sunken Temple or the Old Mine. Each lair has their own unique ally. You can't get them all at the same time.
